ORDER FIXING BAIL

It is hereby ordered that [Defendant's Name], the above-named defendant, be admitted to bail in the above-entitled action in the sum of $[Dollar Amount] cash or surety bond, plus $[Dollar Amount] penalty assessment pursuant to [statute]. In the event that bail is posted by personal bond, the amount thereof shall be twice the amount herein before fixed.
